Samantha Alvarez is entering her 5th year as our office manger for all three locations. She also is responsible for all billing and insurance inquiries. She resides in Fort Wayne with her husband, Dr. Michael Alvarez and their four children.
Mike Hart is a 1997 graduate of Ball State University with a degree in Health Services. He began at Lenscrafters in 1994 as a lab technician. In 1999 he was promoted to General Manager. Mike is a native of Muncie where he currently lives with his wife and two children.
